3. Read the instructions thoroughly and fill the form as prescribed.

4. After you submit the form, a system generated slip will get generated, take print out of that slip.

5. Affix your Stamp size Photograph (recent & colored) at the place given there.

6. Please sign at the concern place.

7. Get the slip attested with any Gazetted Officer or by the H.O.D. of the concern Institute where you studied recently.

8. Send the system generated application form along with your qualifying documents, certificates, age & caste proof etc. and send the same to:
